“Transitions: Putting the Pieces Together” is a one day workshop discussing the extensive value of collaboration and coordination of patient care when transitions are required among home care, care centers, hospitals, physician’s care, senior services agencies, etc.
This event is open to professionals working in Nursing Homes, Adult Care Homes, Community-Based Service Agencies, and other healthcare settings. This event is approved for 6.5 Hours of CEUs from the NC Division of Health Service Regulation Adult Care Home Licensure Section. Application has been made for 6.5 Hours of CEUs from the NC Board of Nursing Home Administrators.
